The Pharmacy OneSource Clinical Team

The Pharmacy OneSource/Sentri7 clinical team represents a collection of over 175 years of diverse experience in providing care, managing healthcare resources and serving the broader patient care needs of hospitals and medical centers. Our team is composed of nurses, pharmacists, infection preventionists and epidemiology specialists with experience in health care technology and quality improvement and certifications in specialty areas of practice.

The knowledge and skill sets of the team combine to ensure we can bring you information on best practices in healthcare and help you implement them into your practice site in a sustainable manner. Our clinical experts work closely with highly specialized software engineering teams, outcomes-focused service teams and consultative sales professionals to ensure a very positive customer experience with world-class, market leading products. Our team is part if the larger Wolters Kluwer Clinical Solutions framework, providing an in-house staff of 70 physicians, 30 pharmacists, and 10 nurses and reimbursement and coding experts, in addition to a 4,000-member external medical content authoring network.

Steven M. Riddle, PharmD, BCPS, FASHP
Vice President of Clinical Affairs

Dr. Riddle serves as Vice President of Clinical Affairs for Pharmacy OneSource/Wolters Kluwer Health. Steve is responsible for strategic planning and clinical marketing and serves as liaison for the company to the healthcare profession.

Steve has over 25 years of healthcare experience including 15 years of clinical practice in med/surg, intensive care, bone marrow transplant, renal transplant, infectious disease and ambulatory disease management services. Dr. Riddle served for 10 years as the quality improvement and medication utilization pharmacist for the University of Washington/Harborview Medical Center in Seattle. In this role he was responsible for the optimal positioning of pharmacy resources to assist the medical center in achieving quality and performance goals.

Dr. Riddle serves as the Chair for the Section of Ambulatory Care Practitioners for the American Society of Health-Systems Pharmacists. He currently acts as a representative for the ASHP on the Pharmacy Quality Alliance. Steve served for 3 years as inaugural Chair for the Washington State Pharmacy Association's Health Systems Academy. He is a member of ASHP, ACCP and WSPA and supports these association's goals of integrating and expanding pharmacy practice to improve patient care. Steve was named a Fellow of ASHP in 2009 and has received distinguished leadership awards from ASHP and WPSA. Mr. Riddle's specific professional interests are quality improvement, health care system reform, and wellness centered healthcare.

Steve received his Bachelor of Pharmacy from Washington State University and his Doctorate of Pharmacy from the University of Washington. He is currently a Clinical Affiliate Professor at the UW School of Pharmacy and serves on the Washington State University College of Pharmacy Dean's Advisory Council.

Janet Blackmere, PharmD
Director of Clinical Solutions

In her role as Director of Clinical Solutions, Janet works with customers in the sales process to understand their workflow issues and provide solutions tailored to meet their needs. As part of this process, she assists customers in building business cases and defining clinical rules, documentation structures and outcomes ("success") metrics. Janet also uses her knowledge around product design and customer needs to direct product development at Pharmacy OneSource.

Janet has 15 years of experience in pharmacy informatics, pharmacy workflow optimization, automation and clinical decision support. She also has 10 years of consulting experience. Janet is a member of American Society of Health-System Pharmacists (ASHP) and the Florida Society of Health-System Pharmacists (FSHP).

Janet received her PharmD from the University of Florida and completed a specialty Residency in Computer Applications to foster Pharmaceutical Care.

Christian Hartman, PharmD, MBA, FSMSO
Director of Clinical Quality and Patient Safety

In his role as Director of Clinical Quality and Patient Safety, Christian is responsible for ensuring the optimal use and development of electronic clinical surveillance and quality improvement solutions. Dr. Hartman also serves as president of the American Society of Medication Safety Officers (ASMSO), a professional organization representing medication safety officers at healthcare organizations. He is an assistant professor of medicine and nursing at the University of Massachusetts Medical School, adjunct assistant professor of pharmacy practice at the Massachusetts College of Pharmacy and Health Sciences, adjunct assistant professor of pharmacy practice at the University of Rhode Island and clinical assistant professor of pharmacy practice at Northeastern University.

Dr. Hartman practiced as a senior research pharmacist at Brigham and Women's Hospital dedicated to human research conducted at Dana Farber Cancer Institute and Harvard University. In 2003 was appointed Medication Safety Officer at UMass Memorial Medical Center in Worcester, MA. Serving as the first MSO, he created the world's first accredited post doctoral residency program in Medication Safety. During his tenure at UMass, the organization had three successful Joint Commission accreditation visits with no medication management RFIs, converted to electronic error reporting, increased error reporting by 150%, and successfully created an electronic solution to detect potential ADEs using the IHI Global Trigger tool and IHI 5 Million Lives: Preventing Harm from High Alert Medications.

He served as the president of Massachusetts Society of Health-System Pharmacists (MSHP) and MSHP Foundation, the ASHP Section Advisory Group for Medication Safety, ASHP Section Advisory Group for Compliance and Quality, University HealthSystem Consortium Pharmacy Council Committee for Medication Use and Technology, and serves on Advisory Board to Decision Health Medication Management Error Elimination. Dr. Hartman received his bachelor of science in pharmacy from the Massachusetts College of Pharmacy in Boston and his doctor of pharmacy degree from the University of Kansas. He completed a fellowship in patient safety at the Virginia Commonwealth University and received a masters of business administration (MBA) program at the Isenberg School of Management at UMass-Amherst. Dr. Hartman is Fellow (FSMSO) of the American Society of Medication Safety Officers.

Harry Jablonski, PharmD
Director of Clinical Operations

In his role as Director of Clinical Operations, Harry works with customers to enhance their utilization of the Pharmacy OneSource products. During implementation, a Pharmacy OneSource clinician is available to work with the customers to help them build a system that works best in their environment and meets their needs. Harry and the Clinical Operations team also support customers with their plans to expand and enhance the Pharmacy OneSource system they have in place.

Dr. Jablonski has over 25 years of pharmacy experience in the acute care hospital, hospital information systems and the pharmaceutical industry. His over 15 years of clinical pharmacy practice includes serving as the Clinical Coordinator and as a Clinical Pharmacist at acute care hospitals in the Midwest. He also served as a pharmacy consultant at McKesson HBOC working with many hospital pharmacies throughout the country to enhance their pharmacy information systems. In the pharmaceutical industry, he fulfilled clinical and business roles at a large healthcare company including responsibilities as a Clinical Liaison, Marketing Manager, and Senior Clinical Consultant.

Dr. Jablonski received his Doctorate of Pharmacy from the University of Illinois at Chicago College of Pharmacy. He is an active member of ASHP as well as a member of the professional pharmacy fraternity Phi Delta Chi.

Tom Jordan, RN, BS, CIC
Director of Infection Prevention

In his role as Director of Infection Prevention, Tom brings his extensive clinical expertise in leveraging clinical electronic surveillance to streamline day-to-day workflow/processes and facilitate real-time clinical interventions leading to the prevention of HAIs.

Tom previously served as the Director of Infection Prevention for a hospital that was one of the initial customers utilizing Sentri7 Infection Prevention. He brings this rich and practical experience along with his extensive infection prevention knowledge to customers adopting the Sentri7 software.

Mr. Jordan has more than 25 years of experience as a registered nurse during which time he provided clinical services in pediatric intensive care, trauma nursing and employee health. Tom served as Director of Infection Prevention for 21 years in acute care and long term acute care settings and is certified in Infection Control and Epidemiology (CIC). His experience includes working in a large, multihospital system (100+ sites). Tom is a long standing member of the Association for Professionals in Infection Control and Epidemiology (APIC) member, an active SHEA member, has contributed articles to the publications Advance for Nurses and Nursing Spectrum, and has presented at national and regional conferences on a variety of infection prevention topics.

Tom holds a Bachelor of Science Degree in Social Work from the Loyola University and a Nursing Degree from Illinois Masonic Medical Center School of Nursing.

Keith H. St. John, MT (ASCP), MS, CIC
Director of Clinical Epidemiology

In his role as Director of Clinical Epidemiology, Keith serves as a clinical consultant to advance informatics solutions to enhance the efficiency, accuracy, standardization, and timeliness of surveillance activities for healthcare-associated infections, while also enhancing patient safety for healthcare settings with limited resources.

Mr. St. John has served as an Infection Preventionist for the past 30 years, including over 17 years of managing Infection Prevention and Control programs and personnel at major tertiary teaching institutions that include pediatric as well as adult hospital settings. Keith is a clinical microbiologist by training and is certified in Infection Control & Epidemiology (CIC).

Keith's rich professional experience includes: Past President of the Certification board of Infection Control & Epidemiology (CBIC); publications in medical and infection control journals; presentation at national and regional conferences; and former faculty associate at Temple Dental and Medical School. Mr. St. John is also a former member of APIC's Governmental Affairs Committee, Education Committee, Practice Guidance Council and Research Foundation. He has served APIC as Chapter President and Board member, Editorial Board and reviewer for AJIC, APIC Text Revision Task Force (x2) and Pharmacy chapter co-author. Keith also completed two five-year teams as a member of the United States Pharmacopeial Convention Expert Compounding Committee (USP 797). In addition to APIC, he is an active member of the Healthcare Infection Society (UK) and the Society for Health Epidemiology of America (SHEA).

Keith received his Master's of Science degree in Clinical Microbiology from Thomas Jefferson University in Philadelphia and his Bachelor of Science degree in Medical Technology from the University of Delaware.

Debra Hagberg, MT (ASCP), CIC
Infection Prevention Clinical Program Manager

In her role as Infection Prevention Clinical Program Manager, Debra supports the development and implementation of clinical initiatives that reinforce the optimal utilization of Sentri7 to meet the needs of our customers and internal team. Her main focus is to provide a successful integration experience and adoption of automated surveillance technology while offering guidance to clinicians regarding infection prevention solutions, healthcare epidemiology, and other IP-related improvement efforts.

Debra has more than 25 years of hospital/clinic infection control, consulting and sales experience. She has 10 years of clinical microbiology experience and 15 years of infection prevention experience in the hospital and clinic environment before she served as a clinical sales specialist and clinical liaison in the infection prevention product manufacturing industry. She also has experience on the business side of electronic infection surveillance technology supporting sales in this industry for several years.

Debra earned her Bachelor of Science in Medical Technology degree from the University of Massachusetts-Dartmouth and she is a member of the Association for Professionals in Infection Control and Epidemiology (APIC). She is certified in infection control and epidemiology (CBIC) and has maintained this certification for twenty years.

Joan N. Hebden, RN, MS, CIC
Infection Prevention Clinical Program Manager

In her role as Infection Prevention Clinical Program Manager, Joan is involved in the development and implementation of clinical strategies and training methodology that support the optimal utilization of Sentri7 to meet the needs of our customers and internal team. She is focused on the successful integration and adoption of automated surveillance and provides guidance to our clinician customers regarding workflow solutions to facilitate achievement of their infection prevention patient safety goals.

Ms. Hebden served as the Director of Infection Prevention and Control for 28 years at the University of Maryland Medical Center, Baltimore, MD. Her clinical background includes general medicine, oncology, and cardiothoracic intensive care. She has presented at national epidemiology conferences, participated in research regarding the transmission of multi-drug resistant bacteria, contributed chapters on infection control to nursing resource textbooks, and published in medical and infection control journals.

Ms. Hebden received her Bachelor of Science degree in nursing and a Master of Science degree in nursing education and trauma/critical care from the University of Maryland School of Nursing. She is certified in Infection Control & Epidemiology (CIC). Joan is an active member of SHEA and APIC and is currently on the Editorial Board of the American Journal of Infection Control (AJIC) and is a faculty associate at the University of Maryland School of Nursing.

Matt Weissenbach, MPH, CPH, CIC
Infection Prevention Clinical Program Manager

In his role as Infection Prevention Clinical Program Manager, Matt supports the development and implementation of clinical initiatives that reinforce the optimal utilization of Sentri7 and meet the needs of our customers and internal team. He is focused on the successful integration and adoption of automated surveillance, providing guidance to clinicians regarding packaged solutions for infection prevention, healthcare epidemiology, and related improvement efforts.

As a trained epidemiologist, Matt has significant experience within the healthcare informatics arena centered on leveraging automation to advance the fields of epidemiology and infection prevention. He has also been extensively involved in international public health, specifically infectious disease epidemiology and vector-borne diseases endemic to Southeast Asia. Matt is a member of the Association for Professionals in Infection Control and Epidemiology (APIC) at the national and local (NC) levels as well as a member of the Society for Healthcare Epidemiology of America (SHEA).

Matt received his Master of Public Health degree in Global Health Practice from the University of South Florida and is board certified in Public Health and Infection Control.
